New Annual Health-Related Services Consent Form

We're reaching out to all Gatesville ISD parents to ask you to complete our new annual health consent form. This is a new requirement for the 2025-2026 school year, based on Texas Senate Bill 12. This law requires schools to get a parent's annual consent for their child to receive routine school-based physical and mental health services from qualified staff like nurses and counselors.

This form helps ensure your child can get timely and equitable access to care that promotes their well-being, safety, and readiness to learn. By completing this form, you're helping us provide the best possible support for all students.

Complete the Form

Please take a moment to complete a separate form for every child you have enrolled in the district. The form is valid for the entire school year unless you choose to revoke it in writing.

Having trouble seeing your child's name?

If you don't see your child's name in the dropdown menu, it's because you are not currently logged into your parent account. Here are the most common reasons and what to do:

  • You've never verified your account: All parents get ParentSquare posts, but to complete forms, you must log in and verify your account first. If you need help verifying your parent account, this Post will walk you through the process.

  • You're logged into a staff account: If you are also a Gatesville ISD employee, you may be logged in to your staff account. Be sure to switch to your parent account to access the form.
